The PharmD perspective
Medication cases move through a continuum.
Understanding what happened can require more than reviewing an order or a prescription. It can require analysis of the clinical decision, the dispensing system, the monitoring process, and the operational setting in which each decision was carried out.
“Technical evidence becomes useful when it is placed in clinical and operational context.”
Clinical intent
Dosing, indication & contraindications
Assessing the clinical rationale, patient-specific factors, and medication risks relevant to the matter.
System review
Interactions, DUR & safeguards
Examining the checks, policies, procedures, and decision points designed to support safe medication use.
Real-world practice

Considering how pharmacy operations, counseling, handoffs, and workload affect implementation.
Ongoing care
Monitoring, reconciliation & transitions
Reviewing the follow-through required for high-risk medications and changes across care settings.
